During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated on November 25, 2024 and completed on November 25, 2024. The on-site inspection began at 9:30 a.m. and ended at 10:55 a.m. The inspector reviewed compliance in the areas listed above. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on December 5, 2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-40-E
REPEAT VIOLAIONT/SYSTEMIC DEFICIENCY The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department. Staff #1 and Staff #2 did not follow procedures in completing the Child Supervision Record (CSR) by checking. A review of CSR document dated November 8, 2024 showed staff not checking out.
Plan of Correction: Teachers retrained on Child Supervision Records best practice on 12/10/24 with teacher sign-off pages indicating completion and comprehension. District Leader will provide in person CSR facilitation on 12/19/24 at monthly staff meeting. Intentional focus on name to face transitions, maintaining ratios, signing in and out as a teacher, and all other layers of the CSR document.

The diapering surface shall be used only for diapering or cleaning children.
A clip board and other small objects was on the diapering surface.
Plan of Correction: Center leaders will visit classrooms periodically throughout the day to assist in keeping diaper changers free of clutter. Teachers were re-trained on 12/10/24 with teacher sign-off pages indicating completion and comprehension.
