An unannounced, on-site focused inspection was initiated on December 16, 2024 and completed on December 16, 2024. The Department issued a NOTICE OF INTENT TO PLACE LICENSEE ON PROBATION AND REQUIRE LICENSEE TO CONTACT PARENTS, GUARDIANS OR OTHER RESPONSIBLE PERSONS REGARDING HEALTH AND SAFETY VIOLATIONS on December 5, 2024 as a result of high-risk violations.The on-site inspection began at 1:50 p.m. and ended at 2:20 p.m. The inspector reviewed compliance in the areas listed above. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
