An unannounced, on-site focused inspection was initiated on January 15, 2025 and completed on January 15, 2025. Department issued a Special Order on January 3, 2025 to PLACE LICENSEE ON PROBATION AND REQUIRE LICENSEE TO CONTACT PARENTS, GUARDIANS
OR OTHER RESPONSIBLE PERSONS REGARDING HEALTH AND SAFETY VIOLATIONS as a result of high-risk violations. The on-site inspection began at 10:10 a.m. and ended at 11:45 a.m. The inspector reviewed compliance in the areas listed above. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
