During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated on February 26, 2025 and completed on February 26, 2025, as a part of probation. The Department issued a Final Order on January 3, 2025 to place the licensee on probation for a period of three months and require the licensee to contact parents, guardians, or other responsible persons regarding health and safety violations as a result of high-risk violations. The on-site inspection began at 12:50 p.m. and ended at 1:45 p.m. The inspector reviewed compliance in the areas listed above. There were 65 children present and eleven staff. This inspection included record review, observations, and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on March 17, 2025. A POC submitted after this date will not appear on the public website.

There shall be at least 12 inches of space between occupied cots, beds, and rest mats. Exception: Twelve inches of space are not required where cots, beds, or rest mats are located adjacent to a wall or a divider as long as one side is open at all times to allow for passage.
In the Discovery Preschool room, two children's cots were touching.
Plan of Correction: Retrain staff on protocols.

If diaper ointment or cream is used, the following requirements shall be met: These products do not need to be kept locked but shall be inaccessible to children.
In the Discovery Preschool room diaper ointment was under the changing table and accessible to children.
Plan of Correction: Retrain staff, licensing checklists three times a day.
