During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site focused inspection was initiated on April 21, 2025 and completed on April 21, 2025, as a part of probation. The Department issued a Final Order on January 3, 2025 to place the licensee on probation for a period of three months and require the licensee to contact parents, guardians, or other responsible persons regarding health and safety violations as a result of high-risk violations. The on-site inspection began at 10:00 a.m. and ended at 1:00 p.m. The inspector reviewed compliance in the areas listed above. There were 54 children present and eleven staff. This inspection included observations, interviews, and a tour of the facility. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
