During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on April 21, 2025 and completed on April 21, 2025. The on-site inspection began at 10:00 a.m. and ended at 1:00 p.m. The inspector reviewed compliance in the areas listed above. There were 54 children present and twelve staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included tour of facility, record review, observations, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on May 1, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-260-A
The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. The provider has no documentation of a fire inspection being completed since 2023.
Plan of Correction: Fire inspection scheduled for 5/8/25.
Standard 8VAC20-780-40-M
REPEAT VIOLATION/SYSTEMIC DEFICIENCY The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.
The allergy list in two rooms was dated February 2025 and the allergy list in one room was dated January 2025. Each list contained information regarding a child that is no longer at the center. One allergy list was hung on the front of a cabinet in the classroom.
Plan of Correction: Corrected on site during visit and will be updated every month moving forward.

Lockdown procedures shall be practiced at least annually. No lockdown drill was completed in 2024.
