Inspector Notes
An unannounced inspection was initiated on September 2, 2025 and completed on September 2, 2025 in response to a self-report received by the licensing office on August 27, 2025 relating to allegations of inappropriate behavioral guidance. The inspector reviewed three staff records on-site. This inspection included record review and interviews. An additional interview was conducted by phone on August 27, 2025. The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
