During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on September 23, 2025 and completed on September 23, 2025. The on-site inspection began at 9:30 a.m. and ended at 11:20 a.m. The inspector reviewed compliance in the areas listed above. There were 54 children present and 12 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 1, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #4 has been attending the center for eight weeks and does not have immunization records.
Plan of Correction: Center will ensure all children have immunizations before start date.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #4, who had been enrolled for eight weeks.
Plan of Correction: Center will ensure all children have physicals on file on start date to prevent this from happening.
Standard 8VAC20-780-40-M
REPEAT VIOLATION/SYSTEMIC DEFICIENCY The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential. In three of the rooms the list were dated differently and contained incorrect information.
Plan of Correction: Moving forward, I have set alarms for the first and Director will be posting them in each classroom.
