During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal/ inspection was initiated on March 18, 2026 and completed on March 18, 2026. The on-site inspection began at 10:10 a.m. and ended at 2:00 p.m. The inspector reviewed compliance in the areas listed above. There were 59 children present and 14 staff. The inspector reviewed six children?s records and five staff records on-site. This inspection included tour of the facility, observations, interviews, and record review.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on March 26, 2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-150-B
Each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department. The immunization and physical record for child #3 did not contain a signature.
Plan of Correction: When collecting immunizations and physical records doctor's signature and date will be checked. The center will check to ensure all immunization and physicals have a signature and date on them.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department. The center did not follow their medication policy to notify parents/guardians about medication that expires soon and returned expired medications to the parent. One medication expired February 2026.
Plan of Correction: Moving forward all medication will be check monthly to ensure none of the medication on site is expired.
Standard 8VAC20-780-40-M
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present. In the Discovery Preschool 1, the list was posted on the white board in the classroom visible to anyone that entered the room.
Plan of Correction: Allergy lists in all classrooms will be kept confidential and not posted in the classroom that is visible to anyone.
Standard 8VAC20-780-570-A
When a child is placed in an infant seat or high chair, the protective belt shall be fastened securely. The four children in Infant 2 were not fastened into their high chairs.
Plan of Correction: Moving forward all infants will be fastened in to high chairs.
