Inspector Notes
An unannounced monitoring inspection was conducted beginning at 12:15 p.m. to 2:30 p.m. with the director. There were 67 children in care ages 4 months through 5 years old under the supervision of 9 staff. Classrooms, supplies and materials were observed. Infants were observed sleeping, being fed, and during transition to and from naptime. Children were observed finishing lunch, during transition time, preparing for nap and napping.
Posted information was reviewed. Five children's records and two staff records were reviewed. Emergency drills were reviewed and up to date.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on observation, the center failed to ensure that all electrical outlets had protective covers. Evidence: In the older infant room, there was an uncovered outlet on the right wall. There was an occupied crib against the wall and the uncovered outlet was accessible to the infant in the crib.

Based on observation, the center failed to ensure that there was at least 12 inches of space between the ends of cribs when occupied. Evidence: In the older infant room, there were four occupied cribs that were touching end to end.

Standard 8VAC20-780-60-A-8
Based on review of documentation and interview with the provider, the center failed to obtain a written care plan for each child with a diagnosed food allergy from a physician. Evidence: Child #1's physical indicated diagnosed food allergies to nuts and eggs. There was no written allergy care plan on file.
