Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on October 21, 2024 as a part of probation. The Department issued a Special Order on June 25, 2024 to place the licensee on probation for a period of twelve (12) months and require the licensee to contact parents, guardians, or other responsible persons (families) regarding health and safety violations as a result of high-risk violations. The on-site inspection began at 9:40 a.m. and ended at 11:50 a.m. The inspector reviewed compliance in the areas listed above. There were 50 children present and 10 staff. The inspector reviewed seven children?s records and four staff records on-site on October 21, 2024. This inspection included document review, tour of the facility and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 31, 2024. A POC submitted after this date will not appear on the public website.
Violations
2SYSTEMIC VIOLATION
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
1. At approximately 9:45 a.m. in the preschool classroom, cereal, cranberries, and a cranberry wrapper were observed on the floor under the tables. At least two areas of spilled milk were observed on top of the table.
2. In the bathroom of the two's classroom, a potty chair was observed with a dried yellow substance all over the bottom on the inside.
In the two's classroom, the lid to the diaper disposal was not functional by foot and had to be lifted by hand to dispose of diapers.
