Inspector Notes

An unannounced inspection was initiated on July 22, 2024, with an on-site visit on August 7, 2024 and completed on August 12, 2024, in response to a complaint received by the licensing office on June 26, 2024, relating to allegations of supervision. This inspection included document review and interviews.

The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to reports and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 23, 2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-80-B-2
Based on document review and interview, the center failed to inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.
Evidence: 1. An injury requiring outside medical attention occurred on June 27, 2024.
2. On July 26, 2024 the center reported the injury to the department.
3. On August 8, 2024, staff #1 confirmed that child #1 received outside medical attention and the center did not report the injury to the department within two business days.
