Inspector Notes
An unannounced monitoring inspection was initiated on October 10, 2024 and completed on November 25, 2024, in response to a self-report received by the licensing office on October 10, 2024, relating to behavioral guidance and forbidden actions. The inspector reviewed one staff record. This inspection included document review and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on December 4, 2024. A POC submitted after this date will not appear on the public website.
Violations
3Systemic Violation
The center is required to follow its own discipline policies. 1. Staff #1 did not follow the center's discipline policy to use positive verbal interactions, as staff cursed at toddlers. 2. Staff #1 did not follow the center's discipline policy that prohibits any kind of physical discipline and pushing. Child #1 received a bloody lip after being pushed on a cot by staff #1.
Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.
Staff #1 cursed at toddlers when they weren't listening.
The following act is forbidden: roughly handling a child. On October 9, 2024, staff #1 pushed child #1 (seventeen months) onto a cot, face forward. This resulted in a lip injury that left visible blood on the cot sheet and child's shirt.
