Inspector Notes

An unannounced monitoring inspection was initiated on January 8, 2025 and completed on January 13, 2025, in response to a self-report received by the licensing office on January 7, 2025 relating to supervision and programs. This inspection was conducted remotely and included document review and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on January 21, 2025. A POC submitted after this date will not appear on the public website.

Systemic Violation
When staff are supervising children, they shall always ensure their care, protection, and guidance. On January 7, 2025, staff #1 observed infant #1 and two other infants trying to climb out of their cribs and did not intervene. Infant #1 (age 14 months) fell from a crib to the ground, receiving a contusion to the head.

Standard 8VAC20-780-440-G
Cribs shall be used for children under 12 months of age and for children over 12 months of age who are not developmentally ready to sleep on a cot or mat. On January 7, 2025, infant #1 (age 14 months), who had predominately been sleeping on a cot for two weeks prior, was placed in a crib for nap. Infant #1 climbed out from the crib, falling to the ground, which resulted in a contusion to the head.
