Inspector Notes

An unannounced inspection was initiated on January 15, 2025 and completed on January 22, 2025 relating to allegations of supervision and ratio. This inspection included document review, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. The violation notice was amended on January 23, 2025.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on January 29, 2025. A POC submitted after this date will not appear on the public website.

The required ratio for children ages 16 to 24 months is one staff per five children. The child supervision record for the toddler room indicates on January 9, 2025, there were thirteen children in the care of only one staff from 1:47 p.m. to 1:57 p.m. and there were thirteen children in the care of only two staff from 2:30 p.m. to 2:51 p.m. (youngest child 16 months old).
Plan of Correction: Completed CSR retraining. Complete manager checks to ensure CSR is correct and lunch breaks do not fall behind

Repeat Violation
Children?s hands shall be washed with soap and running water before eating meals and staff?s hands shall be washed with soap and running water before preparing or serving food or beverages and before helping children with feeding.
1. In the toddler room a child sitting on the floor was moved to the table for lunch without the child?s hands being washed.
2. Without washing their hands, three staff served food and/or beverages to children in the toddler room without washing hands beforehand.
3. Without washing their hands, a staff in the toddler room put food on a spoon for a child and the staff?s hands were not washed beforehand.
Plan of Correction: Completed handwashing retraining with staff, posted handwashing signs in classroom highlighting every time teachers and children should wash their hands
