Inspector Notes

An unannounced monitoring inspection was initiated on May 19, 2025 and concluded on June 24, 2025 in response to a self-report received by the licensing office on May 16, 2025 relating to supervision. This inspection included document review, observations, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 1, 2025. A POC submitted after this date will not appear on the public website.

Children under 10 years of age shall be within actual sight and sound supervision of staff. On May 16, 2025, child #1 (two years old), was left unsupervised and alone on the outside playground for approximately 30 seconds.

Repeat Violation
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance the center?s own policies and procedures that are required by these standards. The center?s procedures for identifying where attending children are at all times includes utilizing Name-to-Face when moving between locations. Before the staff cross the first and last threshold staff are to complete name-to-face checks. On May 16, 2025, Staff #1 and staff #2 did not complete name-to-face checks when moving from the playground to the classroom, leaving child #1 (two years old) alone outside for approximately 30 seconds.
