During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection.
Inspector Notes
The Department entered into a Consent Agreement with the facility on April 16, 2025. An unannounced, on-site monitoring inspection was initiated and concluded on August 28, 2025, with two inspectors present. The on-site inspection began at 10:30 a.m. and ended at 1:20 p.m. The inspectors reviewed compliance in the areas listed above. There were 95 children present and 21 staff. The inspectors reviewed 10 children?s records and 10 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 15, 2025. A POC submitted after this date will not appear on the public website.
Violations
4Training documents for staff #6, #7, and #8 did not include the training delivery method or the number of training hours or credit hours received.
Systemic Violation
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe, and operable condition.
1. In the Pre-k classroom there were numerous flying insects around the trash can and what appeared to be pieces of food on the floor around the trashcan.
2. The toilet in the girl's bathroom in the downstairs hallway had an odor, yellow liquid in several areas on the toilet seat, and used toilet paper under the rim. Staff #12 stated the toilet had overflowed the day before and the toilet was broken.
3. In the DP2 classroom there was visible dirt/crumbs on the diapering table.
Staff shall wash hands with soap and running water before and after helping a child use the toilet. Staff #11 did not wash hands before helping a child toilet.
1. In the DP 2 classroom there were three cups with no name.
2. In the DP1 classroom there were two cups with no names and dates and one cup with the wrong date.
