Inspector Notes

Licensing Inspector Anderson conducted an unannounced complaint inspection in response to allegations received by the licensing office on 2/28/2024, in the areas of sight and sound supervision & infant care. The investigation of the complaint was initiated on March 18, 2024 and findings were reviewed on May 16, 2024.

As the information gathered during the investigation support the allegations, the complaint is determined to be ?VALID.?

Based on observations, the Vendor failed to ensure children less than 10 years of age shall always be within actual sight and sound supervision of staff.

EVIDENCE:
Six children ages 3-5 years old, were observed sleeping in a classroom without sight and sound supervision of a staff member.
