Inspector Notes

An unannounced inspection was initiated on 06/17/2025 and completed on 07/09/2025 in response to a complaint received by the licensing office on 06/17/2025 regarding the building and facility maintenance. The inspector reviewed 3 children?s records electronically on 07/08/2025. The inspection included: document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 07/16/2025. A POC submitted after this date will not appear on the public website.

COMPLAINT RELATED

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

The approximately 5 feet tall plastic partitions used to separate Classroom 1 and Classroom 2 were pushed over by children twice within 30 minutes. The partitions fell over into the play area of Classroom 2 that was occupied by children aged 16 months-2 years.
Plan of Correction: All partitions have now been turned on their sides making them much lower in height and additional legs and brackets have been placed on each one making them sturdy and now able to stand alone.

In areas used by children of preschool age or younger, fans, when used shall be out of reach of children.

There was a fan within the reach of the children (ages 7 months-16 months) in Classroom 1.
Plan of Correction: The fan has been completely removed out of the nursery.

REPEAT VIOLATION

In areas used by children of preschool age or younger, electrical outlets shall have protective covers.

There were 3 uncovered electrical outlets on a power strip in Classroom 1 and an uncovered electrical outlet in Classroom 2. The children in the classrooms were aged 7 months-2 years.
Plan of Correction: Plastic cover outlets have now been placed on all exposed and unused outlets.

Staff administers only medications that were dispensed by a pharmacy and maintained in the original, labeled container.

A portion of Medication 1 had been placed in a liquid medication measuring cup which was being stored in the windowsill of Classroom 1. Medication 1 has been administered by center staff.
Plan of Correction: Medication in cup was discarded. Parental medication authorization has been given by the mother. Only nursery teachers that have been MAT certified are allowed to administer the medicated cream to his skin when needed.

Medications shall be kept in a locked place using a safe locking method that prevents access by children.

Medication 1 was in an unlocked storage bin in the diapering area in Classroom 1.
Plan of Correction: Medicine has been locked properly in storage.

Prepared infant formula shall be refrigerated, dated, and labeled with the child?s name if more than one infant is in care.

There was an unlabeled bottle in Classroom 1 where there was more than one infant in care.
Plan of Correction: All bottles have now been labeled and properly refrigerated.
