Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated on 6/30/2025 and completed on 6/30/2025. The inspection started at 10:40 am and concluded at 12:00 pm. There were 4 children present and 1 caregivers; the point total was 9. The inspector reviewed 2 children?s records and 3 caregiver/household member(s) records on-site.
Violations
10The vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
No attendance was documented for Child #1, Child #2, Child #3, or Child #4 on the date of the inspection.
Caregiver #1 completed 11. 5 hours of annual training.
Two outlets in the bathroom were missing protective covers.
At the time of the inspection, a kitchen drawer near the white freezer contained one large knife, one small knife, and one medium knife. The safety lock on the drawer was broken, making the sharp objects accessible to children.
A bottle of olive oil, perfume, and a container of hair mask cream were located underneath the bathroom sink cabinet, accessible to children present in care.
At the time of the inspection, the bathroom that the children use did not have toilet paper, disposable towels, or an air dryer.
At the time of the inspection, no cots or rest mats were observed in the home. Cargiver #1 stated that the mats were in storage.
At the time of the inspection, Child #1 did not wash their hands with soap and running water or disposable wipes before and after eating their snack.
The first aid kit is missing adhesive tape.
At the time of the inspection, Caregiver #1 did not clean and sanitize the table before and after Child #1?s snack.
