During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 03/30/2026, as a part of the licensure period. The on-site inspection began at 11:40 AM and ended at 1:43 PM. The inspector reviewed compliance in the areas listed above. There were 37 children present and 8 staff. The inspector reviewed 4 children?s records and 5 staff records on-site.
This inspection included:

? tour of the facility,
? interviews, and
? observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include chipped or peeling paint.
The paint along the top rail of a white infant crib is chipped.
Plan of Correction: Facility response
The crib railings will be sanded and repainted or the railing will be covered with a plastic cover made for cribs where children may be teething on the railing.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with the center's own policies and procedures that are required by these standards.
The facility has not followed their own policies and procedures regarding medication as there were two EpiPens and cetirizine stored for emergency administration to children, all of which are expired medications. In addition, there was an EpiPen for emergency administration to child 5 for which there was no written parent authorization.
Plan of Correction: Facility response
All medication policies and procedures will be followed when accepting medication from parents. All staff will be reminded of these procedures.
Expired medications will be returned to the parents.
Standard 8VAC20-780-40-M
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.
There were two children enrolled with life threatening food allergies that were not included on the written list all children's allergies, sensitivities and dietary restrictions.
Plan of Correction: Facility response
The written list of children with allergies, sensitivities and dietary restrictions will be updated and dated. In the future the list will be updated every time a new child enrolls with allergies, sensitivities or dietary restrictions. The staff will be familiar with where this list is kept and will be updated any time changes are made to the list.
