Inspector Notes
An unannounced focused 2 month monitoring inspection was conducted today from 10:40am-11:25am with the provider on site. There were 3 children in care and 2 resident children (11 points) present with 1 caregiver. A sample of 3 children's files and 2 caregiver/household files were reviewed. The children were engaged in the following: playing with toys. Interactions between the children and caregiver were positive. The physical space, attendance
records, drill record and menu were reviewed. There were no medications on site. Areas of non-compliance are identified in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver
names; list caregivers by positions only.
Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

7Based on observation, the violation notice from the last inspection was not posted in a conspicuous place on the licensed premises.
Evidence:
1. The provider had the summary posted from the last inspection dated 5/02/2024, but did not have the violation notice posted.

1. The provider stated Child #3 arrived at 9:00am, but was not listed on the attendance sheet at approximately 10:45am.

Based on observation, outdoor play equipment did not meet the all requirements.
Evidence:
1. The swing seat in the backyard was made of wood and did not have flexible seats of
rubber, canvas or nylon.
Based on review of two children's records and interview, the records did not contain all
required information.
Evidence:
1. Child #3 (start date 6/05/2021) did not have the child's proof of birth, previous daycares listed and authorization for emergency medical care
Based on review, the provider did not obtain the parent's written acknowledgment of the
receipt of the information required in this section.

1. The provider stated she saw Child# 3's (start date 6/05/2021) proof of birth document, but did not document the information and stated the child did not have previous childcares.
