Inspector Notes

An unannounced focused 2 month monitoring inspection was conducted today from 10:40am-11:25am with the provider on site. There were 3 children in care and 2 resident children (11 points) present with 1 caregiver. A sample of 3 children's files and 2 caregiver/household files were reviewed. The children were engaged in the following: playing with toys. Interactions between the children and caregiver were positive. The physical space, attendance
records, drill record and menu were reviewed. There were no medications on site. Areas of non-compliance are identified in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver
names; list caregivers by positions only.
Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Repeat Violation
Based on observation, the violation notice from the last inspection was not posted in a conspicuous place on the licensed premises.
Evidence:
1. The provider had the summary posted from the last inspection dated 5/02/2024, but did not have the violation notice posted.
Plan of Correction: I will make sure I post it.
Standard 8VAC20-800-50-A
Based on review of the attendance sheet and interview, the family day home did not keep a written record of children in attendance each day.
Evidence:
1. The provider stated Child #3 arrived at 9:00am, but was not listed on the attendance sheet at approximately 10:45am.
Plan of Correction: I will make sure I put the time as soon as they come in.

Based on observation, Information contained in a child's record was not kept privileged and
confidential.
Evidence:
1. The supplemental page from the inspection dated 5/02/2024 was posted on
the wall. The supplemental page is considered confidential.
Plan of Correction: I will not post it anymore.
Standard 8VAC20-800-500-D
Repeat Violation
Based on observation, outdoor play equipment did not meet the all requirements.
Evidence:
1. The swing seat in the backyard was made of wood and did not have flexible seats of
rubber, canvas or nylon.
Plan of Correction: I will put the swing out of reach.

Repeat Violation
Based on review of two children's records and interview, the records did not contain all
required information.
Evidence:
1. Child #3 (start date 6/05/2021) did not have the child's proof of birth, previous daycares listed and authorization for emergency medical care
Plan of Correction: I will ask the parents for signature and information.

Repeat Violation
Based on review, the provider did not obtain the parent's written acknowledgment of the
receipt of the information required in this section.
Evidence:
1. Child #3 (start date 6/05/2021) did not have the parent's written acknowledgment of the policy for administering medication.
Plan of Correction: I will ask the parents for signature.
Standard 8VAC20-800-80-C
Based on review, the provider did not document in the child's record the method of verification of the child's age and identity; and the names and locations of the previous child care programs and schools the child has attended.
Evidence:
1. The provider stated she saw Child# 3's (start date 6/05/2021) proof of birth document, but did not document the information and stated the child did not have previous childcares.
Plan of Correction: I will ask the parent's for the information.
