During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site code compliance inspection was initiated on February 10, 2025 and completed on February 10, 2025. The on-site inspection began at 10:25 a.m. and ended at 11:45 a.m.. The inspector reviewed compliance in the areas listed above. There were seven children present with two staff. The inspector reviewed five children?s records and two staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program.

The provider must establish and implement a procedure ensuring that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining sand or other cushioning material under playground equipment.
The dome climber and slide does not have sand or other cushioning material under it. The provider stated they no longer purchase cushioning material for the playground.
Plan of Correction: Cushioning material will be obtained to go under the dome and slide. A procedure will be put in place to maintain the material.
