The center must establish and implement a procedure to ensure that all children in the center are in compliance with the provisions of ? 32.1-46 regarding the immunization of children against certain diseases. Child #5 has been at the center for five months does not have an immunization record on file.
Plan of Correction: The parents of child #5 have been informed to provide an immunization record. In the future the documentation received for child enrollment will be reviewed more carefully to ensure all required documents have been received.
