Inspector Notes

An unannounced monitoring visit and facility tour was conducted from 12:00 pm- 1:15 pm. At the time of inspectors' arrival there were 45 preschool aged children in care with 6 staff members and 1 administrator. Children were observed interacting with staff, nap, lunch and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Injury reports were also reviewed. Director reported that there are no medications being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Based on record review and interview it was determined the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. The record for child #2(under 2 y/o) did not contain documentation of updated immunization records. The documentation present was dated 9/27/21.
2. Staff confirmed this documentation was not available during the inspection.
Plan of Correction: Provider will request this documentation from parent.

Based on observation and interview it was determined the center did not ensure the ratio of 1 staff person to every two year old children was met.
Evidence:
1. There were 9 two year old children in a classroom with 1 staff member.
2. Staff confirmed that the number of children in the classroom exceeded the required ratio.
Plan of Correction: Center will continue to try to hire more staff.
