Inspector Notes

An unannounced monitoring visit and facility tour and investigation of self-reported incident was conducted on 9/26/2023. At the time of inspectors' arrival there were infant to preschool aged children in care with 11 staff members and 2 administrators. Children were observed interacting with staff, lunch and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Injury reports were also reviewed. Director reported medications are not being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Based on record review and interview it was determined the center did not ensure that staff obtained and submit the results of a follow-up tuberculosis screening at least every two years from the date of the first initial screening or testing.
Evidence:
1. The record for staff #1(hired 5/16/22) did not contain documentation of an updated TB test. The test on file was dated 4/22/21.
2. Staff confirmed a more recent TB test/screen was not available during this inspection.
Plan of Correction: Staff will get an updated TB test/screening.
Standard 8VAC20-780-280-B
Based on observation and interview it was determined the center did not ensure that all hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. There were unlocked cleaning solutions in unlocked cabinets in the green and blue classrooms.
2. Staff confirmed these cabinets were unlocked during this inspection.
Plan of Correction: Center staff will make sure all chemicals are locked.
