Inspector Notes

An unannounced, on-site monitoring inspection was completed on 05/21/2025, as a part of the licensure period. The on-site inspection began at 11:50 am and ended at 1:45 pm. The inspector reviewed compliance in the areas listed above. There were 69 children present and 11 staff. The inspector reviewed 5 children?s records and 5 staff records. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Staff #2, who had been employed for on day, did not have a completed national criminal background check.
Plan of Correction: Center will follow up on background check results.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

Several cleaning and sanitizing agents labeled, ?Keep out of reach of children,? were in an unlocked cabinet in the 3-5year-old classroom, within reach of children.
Plan of Correction: Center replaced lock on cabinet.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.

The injury reports for Child #6, #7, and #8 did not contain all of the required information.
Plan of Correction: Center will ensure all required information is on the injury report.
Standard 8VAC20-780-60-A
A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child #2, who has been enrolled for approximately two and a half years, has a diagnosed food allergy and did not have a written care plan in their file.
Plan of Correction: Center requested documentation from parent.
