Inspector Notes

A monitoring inspection was initiated on 6/22/2023 and concluded on 6/22/2023. There were 33 children present with six staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and three staff records were reviewed. The inspection started at 9:40 am and concluded at 12:05 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation the center failed to ensure that the most current violation notice was posted in a conspicuous place on the licensed premises as required by the Superintendent.
Evidence:
1. The Violation Notice dated 11/14/2022 was not posted.

Based on record review the center failed to ensure that staff shall request at least annually parent confirmation that the required information in the child's record is up to date.
Evidence:
Four of the five children's records reviewed did not have annual updates. (records # 2-5)
Plan of Correction: The director will ensure that the parents update the children's files.
