An unannounced inspection was completed due to a self-reported incident concerning supervision of a child. Interviews were held and records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on interview, the center failed to keep children under age 10 years of age always within actual sight and sound supervision.
