Inspector Notes

An unannounced, on-site renewal inspection was initiated on 6/10/24 and concluded on 6/10/24. The on-site inspection began at 9:15am and ended at 12:00pm. The inspector reviewed compliance in the areas listed above. There were 47 children present and 8 staff. The inspector reviewed 5 children?s records and 5 staff records on-site.

This inspection included:
* document review
* tour of the facility
* observations

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-140-A
Each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance.
The file of child #1 did not have a physical, the child has been enrolled and attending the center for 4 months.
Plan of Correction: A copy of the physical will be added to the child's file.
Standard 8VAC20-780-160-C
At least every two years from the date of the first initial screening or testing staff members shall obtain and submit the results of a follow-up tuberculosis screening.
The tuberculosis screening on record for staff #1 was last completed in November 2021.
The tuberculosis screening on record for staff #5 was last completed in May 2021.
Both screenings have since expired and the staff are actively working.
Plan of Correction: Renewed tuberculosis screenings will be obtained and placed in the staff files.
Standard 8VAC20-780-245-L
There shall always be at least one staff member on duty who has obtained, within the last three years, instruction in performing the daily health observation of children.
Review of staff files and discussions with administration during the inspection found that no staff employed and actively working have daily health observation training.
Plan of Correction: Daily Health Observation training will be completed and renewed every 3 years moving forward.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.
There was medication on site, in the active medication lock box, for child B and the parental authorization to administer had expired at the end of April 2024.
Plan of Correction: The parent was contacted and a new authorization was requested. Medication will be sent home today if another authorization is not needed or completed.
Standard 8VAC20-780-550-D
The center shall implement a monthly practice evacuation drill.
Review of documentation and discussions with administration found that there had not been a practiced evacuation drill for April 2024.
Plan of Correction: Monthly emergency drills will be practiced as required.
