Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts and documents review.
A monitoring inspection was initiated on 9/30/2021 and concluded on 9/30/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 9 children present, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-185-160-A
Based on record review, each staff member did not submit documentation of a negative tuberculosis (TB) screening/test within 21 days after employment and not all TB screenings/tests were completed within 12 months prior to employment.

Evidence:
1. Staff #1 (DOH: 9/7/2021) has a TB test/screening on file that was completed on 10/23/2019, the TB test/screening was not completed within 12 months prior to employment.

2. Staff #2 (DOH: 9/7/2021) does not contain a TB test/screening, required no later than 21 days after employment.
Plan of Correction: Both staff were asked to get updated TB test.
Standard 22VAC40-185-260-A
Based on review of records, there was not an updated annual fire inspection available for review during the inspection.

Evidence:
The date of the most recent fire inspection conducted by the fire official was 01/10/2020.
Plan of Correction: Fire Marshall Department was scheduled to visit and provide inspection report after a third party report was not accepted and it became clear what type of report was needed.

Based on record review, staff records did not contain all required information.

Evidence:
Staff #1's record is missing two references as to their character and reputation, as well as competency required prior to the first day of employment.
Plan of Correction: Reference letters delayed after our request.
Standard 63.2(17)-1720.1-B-4
Based on record review, the center did not obtain a fingerprint based background check for each staff member prior to their first day of employment.

Evidence:
The records of Staff #1 and Staff #2 (DOH: 09/07/2021) do not contain fingerprint based background check results required prior to the first day of employment.
Plan of Correction: No information was shared by the previous program coordinator due to her sudden resignation, and it created complications in understanding the process of running background checks,. We ran the background checks at the first opportunity it came to our attention. We submitted the search request and received Fingerprint results for Staff #1 and Staff #2 on 11/22/2021.
