Inspector Notes
An unannounced monitoring inspection was conducted today from 2:00pm-3:15pm. There were 15 children directly supervised by 2 staff. The physical plant, 2 staff records, 2 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed napping, completing worksheets, working on Montessori based activities and practicing the alphabet. There was an adequate supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Violations
7Evidence:
1. Staff #1's (DOD:9/7/2021) record did not contain results of a criminal history result and a sex offender check from a state where the employee has resided within the previous five years. Results were required prior to employment.
2. Staff #1's record did not include abuse and neglect results from a state where employee has resided within the previous five years, abuse an neglect results are required within 45 days of employment.
In this case we sent forms out and are awaiting results, the staff's name in the sex registry came back clean.
Evidence:
Staff #1 (DOH : 9/7/2021) and Staff #2's ( DOH: 9/7/2021) records did not contain Central Registry results.
Evidence:
There was no documentation at the center to verify that they have a written policy and procedures that describe how the center will ensure that each group of children receives care by consistent staff or team of staff members.

Evidence:
There was no documentation at center to verify that the center has a written procedures for prevention of abusive head trauma.
Evidence:
The first aid kit was missing adhesive tape.
Evidence:
There was no documentation to verify that an evacuation drill was practiced in September 2021.
Evidence:
The Violation Notice issued on 9/30/2021 was not posted.
