Inspector Notes

An unannounced renewal inspection was conducted today from 11:00am-12:50pm. There were no children present during the inspection, the program is currently closed. The physical plant, 3 staff records, 3 board officers' records, evacuation drills, emergency supplies, and policies were inspected. There was an ample supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. The center was clean and organized. Information gathered during the inspection determined non-compliances with applicable code sections and violations were documented on the violation notice issued to the facility. Based on record review, the center failed to obtain a copy of the results of a child abuse and neglect search or an equivalent registry from the states in which the staff members have resided in the preceding five years.

Evidence:
1.Staff#1's (DOH:9/1/2021) record does not contain an abuse and neglect search for an state in which the staff member has resided in the preceding five years.
2.Staff#2's (DOH:7/1/2022) record does not contain an abuse and neglect search for an state in which the staff member has resided in the preceding five years.
Plan of Correction: We requested a child abuse and neglect search for Staff #1 and Staff #2, both got delayed.

Based on record review and interview, the center failed to ensure the required background checks were completed for all applicants.

Evidence:
Agent #1 and Agent #2 were designated as board officers in June 2022. On the day of the inspection, the center had already submitted the request, but they had not yet received Central Registry results, which are required before the end of 30 days after the changes.
Plan of Correction: We submitted central registry requests for both agents. We are still waiting central registry results for Agent #2, although we submitted it on July 28, 2022.

Based on record review and interview, the center failed to obtain a central registry check for a staff member within 30 days of employment.

Evidence:
Staff #2's record did contain central registry results. The staff member was hired on 7/1/2022.
Plan of Correction: Central Registry checks were run for NC, FL, and VA. Central Registry checks were received from NC, FL, and VA.
