During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/12/2025, as a part of the licensure period . The on-site inspection began at 12:02pm and ended at 1:45pm. The inspector reviewed compliance in the areas listed above. There were 21 children present and 4 staff. The inspector reviewed 4 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Staff #5, who had been employed for 25 days, did not have a completed national criminal background check.
Plan of Correction: Director will have staff #5 complete background clearance.

The center must request a search of the central registry prior to the employee's first day of employment.

Staff #5, who had been employed for two weeks, did not have a central registry search requested. Staff #5 was working alone with children.
Plan of Correction: Director will have staff #5 submit central registry today.
Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

Staff #5 has been working with children for 25 days and has a TB screening that was dated six months prior to their first day of employment.
Plan of Correction: Director will have staff #5 complete TB screening.
Standard 8VAC20-780-240-B
Staff shall complete orientation training in subsection C of this section prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.

Staff #5 who has been employed for 25 days and was alone with a child had not completed orientation training.
