Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, and interviews.
The Licensing Inspector conducted an investigation on 10/28/2021 in response to a self-report incident of allegations of a unidentified redness on a infant's arm. Interviews were conducted with staff on 10/28/2021. One staff's interview was delayed because of unavailability and completed on 11/17/2021. On 11/3/2021 another public agency investigated the allegations. The information gathered during this investigation does not support the allegations, no violations were cited.
