Inspector Notes

A renewal inspection was initiated on 5/26/2022 and concluded on 5/26/2022. There were a total of 54 children with 10 staff members present. Children were observed during snack time, engaged in group activities, stories, learning shapes and colors. A total of 3 child records and 3 staff records were reviewed. Center policies, fire drills, fire and health inspections were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-160-A-2
Based on a review of staff records, the TB screening documentation has not been completed within the last 30 calendar days of the date of employment and be signed by a physician.
Evidence: Staff #1, date of hire 5/24/2022 does not have documentation of a TB screening. Staff #1 was observed caring for children this date.
Plan of Correction: staff has an appointment today for TB screening.
Standard 8VAC20-780-510-E
Based on a review of medications, the center is not following the procedures for administering medications by not having a parent and physician authorization for long term prescription medication.
Evidence: Child #1 has prescription and over the counter medication for a food allergy, the medication was on site but there was no documentation of parent or physician authorization to administer these medications.
Plan of Correction: Forms will be completed for medications.
Standard 8VAC20-780-550-D
Based on a review of the fire evacuation log and director interview, the center has not completed monthly evacuation drills.
Evidence: There was no documentation of monthly fire evacuation drills for October 2021-december 2021.
The director stated that they were conducted but just not documented.
Plan of Correction: Drills will be conducted and documented on the log.
Standard 8VAC20-780-550-E
Based on a review of the evacuation log and director interview, the center has not completed shelter in place drills twice a year.
Evidence: There was no documentation of shelter in place drills for the year 2021. The last documented drill was dated 12/2020.
The director stated that they have not completed shelter drills in the past year.
Plan of Correction: Drills will be conducted and documented on the log.
