Inspector Notes

A monitoring inspection was initiated on 11/2/2022 and concluded on 11/2/2022. There were47 children directly supervised by 10 staff members. Children were observed during snack time, engaged in group activities and outdoor play time. The physical plant, 3 child records, 3 staff records, medications, evacuation drills, emergency supplies and policies were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on staff record review, at least every two years from the date of the first initial screening or testing, staff members have not obtained and submit the results of a follow-up tuberculosis screening.
Evidence: Two staff have not updated their TB screening every 2 years from their last training. Staff #1 TB dated 10/7/2020
Staff #2 TB dated 8/12/2020
Plan of Correction: Staff will schedule an appointment for their TB screening and have completed by November 18, 2022.
