Inspector Notes
An unannounced renewal inspection was conducted on April 30, 2024, from 9:30am-12:30pm. At the time of inspection, 57 children (4 months to 5years old) were in care with 17 staff members present. The physical plant, 5 children?s records and 5 staff records, 3 children's medications and medication authorization records, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, engaged in group singing and outdoor play time. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children, There was an adequate number of staff with with current CPR and First Aid, MAT, and DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified on the violation notice.
The facility is reminded that a complete renewal application must be submitted prior to expiration of the license on DATE. Renewal applications should be submitted through the online portal available on the public website at childcareva.com.

4Evidence: Reviewed 5 staff files and found that Staff #5, (hire date of 11/27/2023) does not have a central registry finding on file.
Evidence: Staff #5 (date of hire 11/27/2023) has a certificate of Child Abuse and Neglect training dated April 25, 2024.
Evidence: Child #1 was asleep in the crib without a label identifying the crib for use by that child.
Evidence: Child #5 had an authorization from the prescriber with the name brand of medicine needed, the original box of medicine had the generic name.
