Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 1/3/2025 and completed on 1/3/2025, as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as result of the last inspection related to Operational Responsibility and Supervision of Children. The on-site inspection began at 9:35 am and ended at 11:15 am. The inspector reviewed compliance in the areas listed above. There were (47) children present and (12) staff. No children or staff records were reviewed.
