Inspector Notes

This Subsidy Health and Safety Inspection (SHSI) was initiated by licensing staff and concluded on 08/23/2023, between the hours of 10 am to 12 pm. There were 4 children present (10 points), with 2 caregivers supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined (1) non-compliance with the applicable standard. Violations were documented on the Violation Notice issued to the home.

Based on review and interview, lockdown procedures were not practiced at least annually.

Evidence -
1. On the date of inspection (08/23/2023), the posted FDH drill log reflected fire drills and shelter-in-place drills. A lockdown drill was not listed.
2. The Provider stated (during an interview conducted on the date of inspection) that a lockdown drill has not been conducted within the past year.
Plan of Correction: A lockdown drill will be conducted and recorded on the drill log prior to the end of 2023.
