Inspector Notes

This monitoring inspection was initiated by licensing staff and concluded on 03/07/2024, between the hours of 9:30 am to 12 pm. There were 4 children present (11 points), ranging from infant to preschool-age, with 2 caregivers supervising. The children were engaged in the following: music, toy play, games, movie time, diapering, hand washing, nap time, snack, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 6 children files, 3 household records, and 2 staff files were reviewed.

Information gathered during the inspection determined that there were (6) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Standard 8VAC20-800-50-A
Based on review and interview, the Family Day Home (FDH) did not keep a written record of children in attendance each day.

Evidence -
1. On the date of inspection (03/07/2024), the Provider did not have a written attendance for the 4 children in care.
2. The Provider stated that she currently did not document this information.
Plan of Correction: Attendance will be documented on a daily basis. A form was shared with the inspector.
Standard 8VAC20-800-60-B
Based on review, 2 of 4 children files indicated that not all of the required information was contained within each child's record.

Evidence -
1. On the date of inspection (03/07/2024), the policy name for the medical insurance for Child #1 was not available for review.
2. There was 1 of 2 phone numbers and 0 of 2 addresses documented for the 2 emergency contacts listed for Child #4.
Plan of Correction: All missing information will be obtained and placed on file.
Standard 8VAC20-800-70-A
Based on review, 2 of 4 children files indicated that there was not documentation in place to establish whether or not the Family Day Home had liability insurance (of at least $100,000 per occurrence and $300,000 aggregate in force on the FDH operation) as required by ? 22.1-289.050 of the Code of Virginia.

Evidence -
1. On the date of inspection (03/07/2024) a notice of liability form had not been completed for Child #1 (start date - 07/14/2022), and Child #4 (start date - 10/03/2021).
2. This was confirmed on the date of inspection by the Provider.
Plan of Correction: The liability form will be completed and placed on file for all applicable children.
Standard 8VAC20-800-780-B
Based on review, 2 of 4 children files indicated that an annual review of each child's emergency information and contacts was not conducted.

Evidence -
1. On the date of inspection (03/07/2024), the most recent annual review for Child #1 was conducted on 07/14/2022.
2. The most recent annual review for Child #4 was conducted on 10/03/2021.
Plan of Correction: All applicable reviews will be conducted and verified with a parent signature and date.

Based on review, documentation for the age and identity for 1 of 4 children in care was not established within 7 business days of each child's first day of attendance.

Evidence -
1. On the date of inspection (03/07/2024), a proof of identification was not available for review for Child #5.
2. The start date for Child #5 was listed as 05/31/2023.
Plan of Correction: Proof of id for Child #5 will be documented and placed on file.
Standard 8VAC20-800-90-C
Based on review, 1 of 4 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.

Evidence -
1. On the date of inspection (03/07/2024), the immunization documentation on file for Child #2 was dated - 11/14/2022.
2. Based on the information on file, on the date of inspection, Child #2 was under 2 years of age.
Plan of Correction: An updated immunization page will be obtained and placed on file.
