Inspector Notes

An unannounced renewal inspection was conducted today from approximately 1pm-3:30pm. There were 57 children in the direct care and supervision of 20 staff. A sample of 4 children?s records, and 4 staff records were reviewed. The children were observed napping, playing, and being picked up by parents. Two new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the Owner. Based on record review, a sex offender search was not conducted for staff whom live outside of Virginia within the last 5 years.

Evidence:
A sex offender search was conducted for Staff A and Staff B.
Plan of Correction: The sex offender searches for Staff A and B were completed during the inspection.

Based on observation, hazardous substances were not kept in a locked place.

Evidence:
Cleaning supplies were located in a cabinet located on the wall of each room.
Plan of Correction: Locks will be applied to each cabinet.

Based on record review, the record of medication administration was missing required information.

Evidence:
The record of medication administration was incomplete for two children.
Plan of Correction: Staff will be reminded to complete the entire record.

Based on record review, documentation of the emergency drills was not maintained.

Evidence:
Documentation for emergency drills conducted in 5/2021, 6/2021 and 7/2021 were missing from the document provided to the inspector.
Plan of Correction: Moving forward the center will use the DOE model form to document emergency drills.
