The center is required to obtain a completed sworn statement prior to the employee's first day of employment.

The sworn statement for Staff #1 (employed over 9 months), was dated two weeks after their first day of employment.

Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

-The national criminal background check for Staff #2 (employed over 4 years), was dated over 20 days after their first day of employment.
-The national criminal background check for Staff #4 (employed over 2 years), was dated over 60 days after their first day of employment.

The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.

The immunization record for Child #5 (enrolled over 10 months) was overdue by 5 months.

Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.

Staff #4 (employed over 2 years) completed this course over 8 months after employment, which was overdue by more than 5 months.

REPEAT VIOLATION
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

-Classroom #6 had hand sanitizer unlocked and within reach of children on a teacher?s desk and 3 full bottles within reach inside of a cubby.
-An unlocked bin of cleaning supplies was present on top of the children?s cubby area in the hallway outside of Classroom #6.

The center may administer prescription medication if dispensed from a pharmacy and maintained in the original, labeled container.

Medication #2 and #3 were not in the original container with the prescription label.

REPEAT VIOLATION
When food is brought from home the food container must be clearly dated and labeled in a way that identifies the owner.

The lunch container for Child #2 was not dated or labeled in a way that identified the owner, and the lunch container for Child #3 was not dated.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The record for Child #5 (enrolled for over 10 months), was missing the name, address, and phone numbers of two designated people to call in an emergency if a parent cannot be reached, and documentation of viewing proof of the child's identity and age.

REPEAT VIOLATION
Each staff record will contain documentation of health information on a staff?s physical and mental health.

The records for Staff # 2, 3, and 4 did not contain health information.
