Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A monitoring inspection was initiated on 08/11/21 and concluded on 08/16/2021. The director was contacted by telephone to initiate the inspection. There were 23 children present and 5 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a record review the center did not ensure each child had a physical examination and immunizations under the direction of a physician.
Evidence:
Child #1 , enrolled 07/19/21 did not have documentation of a physical examination or immunization record signed by a physician.
Plan of Correction: The following corrective action was provided by the director during the exit interview:
The parent pulled the child's record from an online database from the child's physician. The director will request a signed physical and shot record from the parent today.
Standard 22VAC40-191-60-B
Based on a record review the center did not ensure all sworn statements of affirmation were signed prior to staff's first date of employment.
Evidence:

Staff #1's first date of employment was 06/01/21 and the sworn statement of affirmation was signed and dated on 06/02/21.
Staff #3's first date of employment was 07/29/21 and the sworn statement of affirmation was signed and dated on 08/02/21.
Plan of Correction: This was an oversight. All sworn statements will be signed prior to any new staff beginning employment with the center.
