Inspector Notes

An unannounced inspection was initiated on 3/13/25 and completed on 6/9/25 in response to a complaint received by the licensing office on 3/12/25 relating to allegations of behavior guidance. The inspector reviewed 2 children?s records and 1 staff record on-site. This inspection included document review, interviews and observations. The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/16/25. A POC submitted after this date will not appear on the public website.

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior. On 3/7/25 staff #1 did not use appropriate behavior guidance when guiding the behavior of child #1, a two-year-old. While at a table, staff #1 stood up, quickly brought her hand down to the table and said ?You?re going to sit right by yourself if you keep it up?. Staff #1 said she hit the table with a flashcard and told the child "I'm done with you". Staff #1 stated ?my tone may have been a little louder. I may have been overbearing,?
