Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Staff #1, began employment prior to receiving the criminal history results.

The center must obtain a copy of the results of a central registry check. Staff #3, who had been employed for 12 months, did not have a central registry results in the record.

Repeat Violation
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. Staff #2, employed for three weeks, did not have documentation of requesting a sex offender and child abuse and neglect registry check from another state.

Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #2, who had been enrolled for three months.

Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department. The immunization record for child #2 was not signed.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1, enrolled for four months, was missing the following required components: Two emergency contacts. The record for Child #2, enrolled for three months, was missing the following required components: Two emergency contacts and place of employment for each parent.

Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment. Staff #1 and Staff #2 did not have any completed reference verifications on file.
