Inspector Notes

An unannounced, on-site monitoring inspection was completed on 09/13/2024. The on-site inspection began at 10:00 am and ended at 12:50 pm. The inspector reviewed compliance in the areas listed above. There were 90 children present and 20 staff. The inspector reviewed 8 children?s records and 5 staff records on-site. This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/24/2024 POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-160-C
Each staff member shall submit documentation of a negative tuberculosis screening at least every two years from the date of the first initial screening.

The screening for staff #1 was expired for 5 days. The screening for staff #2 was expired for 2 1/2 weeks. The screening for staff #4 was expired for 6 weeks.
Plan of Correction: Teachers were sent to update their TB tests. A better monitoring expiration date was put in place.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of the occurrence. The record shall include any action to prevent future reoccurrence of injury.

Two injury reports did not contain written record on actions to prevent future reoccurrence of injury.
Plan of Correction: The action plan to prevent future reoccurrence of injury will be added to the electronic system.
