Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the program.

A code compliance inspection was initiated on July 30, 2021 and concluded on September 10, 2021. The director/in-charge person was contacted by telephone and a virtual inspection was conducted. There were 50 children present, ranging in ages from infant to school age, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of 10 staff records were reviewed.

Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the program.

Based on record review the vendor failed to obtain a central registry background check within the allotted time frame.
Evidence:
1 - The record for staff 2 does not contain any documentation that a central registry background check was requested or returned to the center.
a. Staff 2 has a documented date of hire as March 9, 2021.
2 - Staff 1 confirmed that there is not a central registry background check on file for staff 2.
Plan of Correction: We will make sure that we are adequately trained on background check regulations. We will follow-up to be sure that we have received all requested background checks.
