An unannounced code compliance inspection was initiated on September 25 and concluded on September 26, 2023. There were 56 children in care ages two months through five years old with 10 staff. Children were observed in teacher and self-directed activities. Staff background checks and health reports as well as children's immunizations were reviewed.

Based on record review the center failed to ensure each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision.
