Children's records shall include all required information. Seven out of seven children's records reviewed do not include emergency contact addresses as required.

Staff records shall be kept of each staff person and include date of employment. Six out of eight staff records reviewed do not include a date of employment.

Staff records shall include tuberculosis screening results. Five out of eight staff records reviewed do not have any documentation of tuberculosis screening results.

Staff records shall include date of separation from employment. Two out of eight staff records reviewed did not included documentation of the date of separation form employment.

All staff shall have current certification in cardiopulmonary resuscitation (CPR). Two out of eight staff records reviewed do not contain any documentation that CPR certification was obtained as required.

All staff shall have current certification in first aid appropriate to the ages of children in care. Two out of eight staff records reviewed do not contain any documentation that first aid certification was obtained as required.

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition. There are wooden cubbies throughout the center that are not secured and pose a toppling hazard. There is a top loading freezer accessible to children that does not have child proof latch and poses an entrapment hazard.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. There was an unlocked cabinet in the toddler/2year old bathroom where cleaning and sanitizing chemicals were stored within reach of children.

Medication, except for those prescriptions designated otherwise by written physician's order, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children. There is a medication on shelf , an unlocked location, in a classroom within reach of children in care.
